I was not in a position to afford or wait for a appointment with my doctor. It was reasonably priced and i received a refill for my needed Rx to hold me until I could reach my Primary care doctor.

When you have two mental health providers who refuse to sign your FMLA because they are entirely ignorant of the guidelines laid out by the Department of Labor, you might feel a little betrayed and desperate. I felt like I was having a week-long panic attack until I found Dr. Sartin. Finding a provider that understood things like 'FMLA eligibility' and who could certify the WH-380E was such a relief. Dr. Sartin truly saved me. I was in total fear of having to abandon my leave of absence and return to work in the state I was in, when work was the reason I took the leave in the first place! I hope this review finds anyone who has experienced the utter nonsense I have with these local providers. Dr. Sartin and her team can help! Best money I have ever spent...

I reached out to Dr. Sartin after weeks of struggling to get medical leave paperwork completed for my Short-Term Disability. I had undergone multiple dental procedures — a root canal, tooth extraction, dental implant, fillings, cleanings, and even teeth whitening — and the pain, follow-up appointments, and healing process made it nearly impossible to keep working. On top of that, I was mentally and emotionally exhausted from trying to manage everything without the proper support.

I originally tried to get my dentist to complete the paperwork, but it was a stressful and time-consuming process that didn’t give me the results I needed. I went back and forth just trying to get basic documentation submitted, and in the end, the paperwork was incomplete. It caused delays, confusion, and a lot of anxiety — especially since I was already struggling physically and financially.

That’s when I finally found Dr. Sartin. I honestly wish I had reached out to her from the beginning. She listened without judgment, understood my situation right away, and completed everything I needed within two days. My leave extension was approved just in time, and I was able to get the Short-Term Disability pay and protection I needed — all without the stress I’d experienced before.

What makes this especially important is that many people don’t realize you can take protected medical leave for dental issues, especially when they’re severe. In my case, the procedures — and the stress that came with them — were real, and Dr. Sartin helped validate that. She made the process simple, supportive, and fast.

Such a great service, especially when time is of the essence! Everything went as smooth as ever and much better than even I expected. If you are fearful or hesitant about using this service as a valid FMLA resource then think again.

I booked my appointment and filled out the intake form explaining my medical condition and answered a few questions pertaining to the severity of my symptoms. Even though I had misunderstood a question or two it was very easy to correct my mistakes via the very courteous and professional Assistants using a chat service.

Dr. Sally was very patient and understanding. When our appointment finally came she asked a few more questions and reviewed my intake form and made sure I qualified for FMLA based on my symptoms. I came out of the experience hopeful and ready to start treatments knowing that the hard part of going through a lengthy process of receiving, filling out, and submitting paperwork was over.

Absolutely would recommend Dr. Sally and this service for anyone who needs FMLA paperwork but doesn’t have the time to go through the regular process of being shuffled around or requiring multiple appointments. Just make sure you qualify both with your employer as well as your symptoms/condition.

Dr. Sally Sartin is the absolute best MD I have ever had. She was very attentive and reviewed my concerns with care. Scheduling was easy and quick. Dr. Sartin was so nice and caring. I had such a positive experience that I will be using her services again and I am going to be allowing her to become my PCP to help streamline my care. All doctor offices should model themselves, their care, and practice after Dr. Sartin! She is fantastic and great at what she does. My FMLA was approved within 2 days of submitting my paperwork. She handled my complex situation with care so that I could focus on getting better. I look forward to a successful healing journey. This was the best medical experience ever and my care requires me to see multiple specialist for my health situation and I have never had such a wonderful experience in my life with a Doctor. She truly changed my life.

I was honestly skeptical about this until I saw the reviews. However, this is VERY LEGIT! I had an appointment and it went smoothly. I uploaded my documents after the appointment and was in constant communication about the paperwork. I received it a couple days later. I waited until I was approved to write this review. I was approved for FMLA for the year. Definitely worth the cost as it is so hard to find a doctor on a quick notice to fill out paperwork or even get an appointment. Thank you Dr. Sartin!!

I was so frustrated trying to get an appointment to speak with a provider who could fully help me, but got no where.

After a little research, I found and called My Doctor's Note, aka HealthSource Medical Associates and actually spoke with a person who answered my questions. After that I felt comfortable making an appointment.

I met with Dr. Sartin, he was amazing. He empathically listened to all my concerns and gave me great recommendations. I felt understood and finally got the help I needed. He submitted all my docs very quickly and I was approved for my leave.
